Description
The Individual Support Model includes the methodology and working tools necessary for the completion of Individual Support Plans (ISP) designed for people working in Lantegi Batuak with mental disabilities, traumatic brain injury and mental illness, and are designed to:
• improve the attention span of people with disabilities
• respond to the personal expectations of each of these people
• improve their quality of life and levels of satisfaction
Each Individualised Support Plan includes the activities, services and support that Lantegi Batuak will carry out or that the person must take in order to achieve the personal goals they have set. It also includes the assessment of their personal profile, training objectives, necessary adjustments to the workplace and any other support that person requires.
In 2010, after several years of implementation, we have conducted a review of the methodology of the Individualised Support Plans which currently contains a more precise definition of all the specific aspects covered by the ISP.
